Athletic Code Packet

Participation in the Athletic Program at Temecula Prep is a priviledge and a responsibility.  This packet explains exactly what is required of all student athletes and their parents. Download this packet and submit the appropriate forms prior to participation.

Physical Exam Form

An annual physical examination by a physician certifying that the student is cleared to participate in athletics is required.  Download and take this form with you to the physical exam for the doctor to fill out.  This form must be submitted prior to participation.

TPS Classic Tournament Sponsor Form

Be a Patriot, Liberty, or other level sponsor and get tournament signage for your business, along with other gifts and incentives.  We would be priveleged to have your company help us raise funds for our Athletic Program.  See form for contact information, and sponsorship specifics.
